What is community rehabilitation?

Community rehabilitation focuses on helping people function better in their own homes and local communities. Our services support you to achieve your personal goals—whether that’s returning to work, managing daily routines, reconnecting socially or increasing your overall physical and emotional wellbeing.

We work with children/tamariki, young people/rangatahi, adults and older people to help them overcome barriers and thrive in their everyday environments.

What we offer: Our range of rehabilitation services

Our community rehabilitation services include home-based rehabilitation, functional and cognitive assessments, return-to-work support, speech therapy and social re-engagement — all personalised to your unique needs.

Home-based rehabilitation.

Individualised rehabilitation is delivered in the comfort of your own home. This can include mobility support, personal care planning and daily living skill retraining.

Functional assessments.

Evaluations to understand your current abilities, limitations and goals are used to guide effective, person-centred rehabilitation plans.

Cognitive and emotional rehabilitation.

Supporting those recovering from brain injury, concussion or neurological conditions, these services help with memory, attention, problem-solving and emotional regulation.

Return to work support.

This support helps you rebuild confidence and functioning in the workplace through gradual return-to-work plans, job coaching and liaison with employers (in coordination with ACC or funders, where appropriate).

Speech and language therapy.

These therapists can provide support for communication or swallowing difficulties resulting from injury, illness or neurological conditions.

Community access and re-engagement.

These services help you to reconnect with social, recreational and community activities that are meaningful to you.

Training for Independence (TFI).

TFI helps you develop or re-learn everyday living skills, improve your confidence and mobility, manage emotional and psychological challenges and reintegrate into your home, workplace or community. We hold the Te Ata Tu, Te Ata Pō and Tamariki and Rangatahi contract.

Injury-related support services.

After injury, these support services help you rebuild confidence and function. We can provide integrated recovery support, including physical rehabilitation, emotional support and integrated recovery planning.

Who can access this service?

You may benefit from Geneva Wellbeing’s Community rehabilitation services if:

  • you’re recovering from a concussion, traumatic brain injury (TBI) or other injury
  • your ability to perform daily tasks has been affected
  • you need support regaining confidence, skills or structure in your daily life
  • you’re looking to return to work, education or social activities
  • you’ve been referred by ACC or a healthcare provider for injury rehabilitation.

No, you don’t need an ACC claim or GP referral to get support from Geneva Wellbeing. While many of our clients are referred through ACC or health providers, we also welcome private clients who want to access support directly.

Yes, we offer home-based rehabilitation as well as clinic and telehealth appointments. Our goal is to make rehabilitation as accessible and effective as possible—delivered where you feel most comfortable.
