What is a wellness assessment? A wellness assessment is a personalised check-in that looks at your overall health and wellbeing. It helps you understand your current mental and physical health, what support you might need, and how to make positive changes in your daily life.
Who can have a wellness assessment? We work with individuals of all ages — from children and young people to adults and older people. Whānau can also book wellness assessments to get support for a loved one or the whole family.
What does a wellness assessment include? Your assessment may include a conversation about your physical health, mental wellbeing, lifestyle, and any concerns you have. Depending on your needs, it might involve screening for stress, fatigue, anxiety, or other challenges. We create a clear plan with recommendations and next steps.
Is it covered by ACC? Some wellness assessments may be funded by ACC if they relate to recovery from an injury, concussion or mental health concern linked to an ACC claim. We can help check your eligibility. If you’re not covered, you can still access our services privately.
How much does it cost? If your wellness assessment is funded by ACC, there is no cost to you. For private assessments, costs vary depending on the type and length of service — we’ll explain all options upfront so there are no surprises.
Where does the assessment take place? We can meet you at home, in your community, at one of our clinics or online — whatever works best for you and your whānau.
What happens after the assessment? You’ll receive clear feedback and a plan with practical recommendations. If you’d like, we can also support you with follow-up services such as therapy, coaching or rehabilitation.
How do I book a wellness assessment? You can be referred by your GP, an ACC case manager or self-refer at any time. Just get in touch with our team and we’ll guide you through the next steps.
